The DOJ cannot use chronic understaffing, ignored procedures, malfunctioning equipment, and falsified records to explain his death while allowing many of those same problems to persist years later. “Systemic breakdown” has become a convenient way to spread blame so widely that almost no one is held meaningfully responsible. The government acknowledged enough institutional failure to defend its conclusion, but not enough to force the institution to change.
